
Person and Team Centered Plans
A process that engages family and district teams to identify gifts and capacities in a student or adult, as well as dreams and possibilities for the future, in order to make a positive plan to transition to the next phase of life. Person-Centered Plans focus onstrengths, solutions, and teamwork and are a great resource for IEP teams.
Person Centered Plans focus on a student's strengths and can be administered at any age, at any transition point. The process includes the voices of the student, school staff, friends, and family, in order to embody and celebrate the whole child.
Team Centered Plans follow the same strengths-based process, and are an effective strategy for gathering information and planning for next steps when a team is in transition, has new members, or is looking to build capacity.
